Incorrect. Time Lords have 12 regenerations, thusly 13 incarnations. Assuming William Hartnell is the first incarnation (or even John Hurt), and John Hurt is the only "forgotten" incarnation, he has 1 more regeneration. Wut?

Gotcha - well that explains the regeneration rumored to happen mid Series 8. And the way I hear it, John Hurt plays the regeneration before Eccleston - a version of The Doctor that did things during The Time War so wrong that he has chosen to forgotten about that period until now.

Thats feasible. Only regens not seen onscreen ate leading into Hartnell (if there are any pre-Hartnell) and McGann to Eccleston.

One thing to consider (Spoilers?)...
...Since The Doctor is the only Time Lord left (at the moment, unless the time lock gets undone again or the weasel The Master back into things), who's to say that he even has a limitation on Regenerations anymore? Everything has to die eventually-certainly the Doctor is no exception-but if he can get around so many other things, who's to say that he can't get past this? Furthermore, notice how they haven't really played up just how many times he can regenerate recently...
...Just saying, there's some tomfoolery afoot.
